Birth & Death Certificates
To receive a copy of a birth or death certificate, please complete the appropriate form
below and either mail it to us or bring it to our office. Our address is:

Renovations are moving forward at the Health Department building at 1650 Boone's Lick Road in St. Charles. During the project, many services will be temporarily relocated to accommodate construction. Here is the latest information as of January 28, 2008:
Vital Records (Birth and Death Certificates) Services have temporarily moved to the lower level of the new administrative area. Patrons can access Vital Records through the doors on the southwest corner, closest to Boone's Lick Road. Signs are posted to direct you to the new location.
